Council on Aging is partnering with the Live Well Collaborative to better understand and improve how older adults and caregivers access information, resources and services through our organization. information, resources and services for older

adults and their caregivers who want to remain independent in their homes and communities.We’re conducting a community survey to better understand how older adults and caregivers connect with us to find information, and learn about or enroll in our programs. Please help us by completing a brief survey.
