For Sedgwick to provide LACC members with a quote, Sedgwick needs to file a BWC Temporary Authorization to Review Information form (AC-3). This Ohio Bureau of Workers’ Compensation (BWC) form allows us to evaluate your prospective members for placement in your group program.
